Employee Requirements

We at St. Rose CYO strive to create a safe environment for all our parish and community members.


As directed by the 'Diocese of Rockville Centre - Office for the Protection of Children and Young People,' all St. Rose CYO employees must comply with the U.S. parish and diocese specified safety practices and guidelines.


All St. Rose CYO employees (referees, scorekeepers, security, etc.), must complete the following three (3) requirements of employment prior to their start date; 


 1. Complete the Employee Information form. Click link CYO Employee Information.

 2. Complete the Request for Taxpayer Identification Number and Certification (W9) form (see PDF below) and mail page one to St. Rose of Lima CYO,  2 Bayview Ave, Massapequa, NY. 11758.  Signature and SSN are required for form to be valid.

 3a. If you are 18 years of age or older, the following three (3) requirements must be completed online through the Virtus system (www.virtus.org). See PDF attachments below for instructions on how to use Virtus for both new and existing account holders.

  1. Attend Protecting God's Children Program; and
  2. Sign the Codes of Conduct; and
  3. Complete a background screening application



 3b. If you are Under the age of 18, read and electronically accept the St. Rose CYO Code of Conduct. Clink link St. Rose CYO Code of Conduct (U18).
